The Finest Citizen Award - Nomination Form

The Town of Wasaga Beach's Finest Citizen Award recognizes individuals or groups who demonstrate outstanding volunteer contributions across multiple areas within the community. Nominees must be residents of Wasaga Beach. While individuals or groups may be nominated multiple times, they are eligible to win the award only once. There are no age restrictions for this honour. Please note that only information included on this form will be considered in the award evaluation and selection process.

Nominator Information

Please answer the following questions and provide relevant information as to why you are nominating this individual for the Finest Citizen of the Year Award. 

• Testimonials – Letters of support from individuals who are familiar with the nominee's community involvement.
• Clippings – Newspaper or journal accounts that highlight the nominee's commitment.
• Photographs – Useful for illustrating the nominee's involvement and any previous awards or recognition.

Personal information contained on this form is collected pursuant to The Municipal Freedom of Information and Protection of Privacy Act, and the Municipal Act, 2001, as amended.
